In 1869 the Willard State Hospital opened on the shore of Seneca Lake in New York, the first mental asylum built expressly for the chronically insane. By 1875 there were a thousand patients crowded into this asylum of no hope (Grob, 1994). At Willard, and at other asylums where the patient populations had far outgrown the facilities and the staffs, treatment effectively ceased. Staff efforts were directed primarily at patient management. At the end of the 19th century, the patient population in American asylums underwent another period of rapid growth as these asylums were increasingly used as homes for the elderly, especially those suffering from dementia.
