It is apparent that changes in thought content and process with mania lead to changes in behavior. These behaviors can initially be quite positive. Some people report that they are quicker, physically and mentally, when mania is beginning to emerge. If not impaired by distractibility or racing thoughts, they can be more efficient than usual, able to multitask, and have the stamina to put in the added time needed to take on new activities or persist at old ones. Unfortunately, the activities stimulated by mania are not without problems.

And community mental health centers rarely have the resources to provide psychotherapy for the people they serve. Therefore, the cognitive-behavioral interventions described throughout this book should be thought of as adjunctive to ongoing treatments. Given this framework, how can CBT help people who suffer from bipolar disorder? When the first edition of this book was published, psychotherapy for bipolar disorder was a new concept. It had previously been believed that a psychosocial approach to the treatment of bipolar disorder was a waste of time.

In contrast, the mood changes in mania can be positive, hopeful, excited, euphoric, “on top of the world,” or exhilarated. For few patients are manic episodes always pleasant. Most have had periods of feeling extremely irritable, agitated, anxious, tense, and fearful. For some, the pleasant or euphoric mood evolves into irritability as the mania progresses and worsens. In mania, the cognitive changes also vary greatly across patients but generally consist of changes in cognitive processing, changes in quality of cognitions, and changes in thought content.
